Impact of the pre-analytical phase on enzymatic homocysteine determination

This work deals with the stability of homocysteine in blood plasma, it emphasizes the importance of the preanalytical phase of laboratory examination, from the preparation of the patient for blood collection, to the insertion of samples into a fully automatic analyzer. The theoretical part describes homocysteine as such, the diagnostic significance of its examination and describes the laboratory environment. The experimental part monitors the concentration of homocysteine in samples that were processed after a different time interval from blood collection. The measured values of plasma homocysteine concentrations showed that with increasing time lag between sampling and processing itself, the stability of the analyte to be determined is impaired. The stability of plasma homocysteine was also verified in samples processed immediately after blood collection and left at room temperature.

13 C-MTG breath tests are used for exocrine pancreas diagnostics. It is a quite expansive test and its usage has never been tested in clinical practice in long time horizon. This study is engaged in pre-analytical phase and contains a group of 4 people who were tested for 6 months and this is the first study ever that has been proceeded in a long time horizon. The half-year profile of these 4 people showed that some of those results are spread of tens percents against its average. The test of CO2 stability showed that the value of CO2 is independent on the temperature which the breath bag is stored in. Its concentration was same for the time of 7 days. Now we can say that the breath bags are insensitive in case of transporting and storage. The question is, why the value of delta 13 C/12 C changed. The results of this study point on fact that values of cumulative dosage recovery of these 4 people did change.
Influence of the pre-analytical phase on glucose and potassium blood examination

This bachelor thesis deals with the stability of glucose and potassium in blood. The main objective of this work was to test the stability of both analytes with regard to the prolonged time from sampling to separation and the temperature conditions of sample storage before the actual analysis. For glucose, stability was tested in two different materials, both in serum and plasma, with the addition of a glycolysis inhibitor.
Elements of the Preanalytical Phase of Blood Taking Influenced by Nurse and their Adherence in Practice.

The bachelor thesis is divided in theoretical and empirical part. The theoretical part deals with preanalytical variability influencing blood test results with a closer view of elements that can be affected by a nurse. The empirical part consists in quantitative research using questionnaires which determine the nurses's awareness about this issue. Qualitative research focuses on the use of this knowledge in nursing practice by methods of observation. The aim of this research was to investigate the knowledge level of nurses in the issue of preanalytical phase of blood sampling and to confront theory with practice. Subsequent comparison of the results of both methods showed that the commonly usedpractice is not always consistent with theoretical knowledge. The most important discrepancy between theory and practice has been established in connection with the preparation of the patient, the duration of application of a tourniquet, the flexion-extension arm exercise prior to sampling and shaking of test tubes containing blood. Investigation of markers for the diagnostics of metabolic syndrome

In my Bachelor´s thesis I dealt with the issue of investigation of biochemical markers, which are used for diagnostics of metabolic syndrome along with measurement of waist circumference and level of blood pressure. In the theoretical part I defined the term metabolic syndrome. Chapters about glucose and blood lipids tests as well as methods used for their determination are described in detail. Great attention was also paid to preanalytical factors, which significantly share of the accuracy of laboratory test. Practical part of the Bachelor´s thesis is focused on acquisition of practical skills in work with biological material in the laboratory of clinical biochemistry and acquisition of principals of laboratory methods. Under professional supervision in the biochemical laboratory Laboma s.r.o. in České Budějovice, I tested 100 samples of venous blood, where I determined concentration of fasting glucose, triglycerides and HDL cholesterol. The measurement was carried out on the biochemical analyzer Cobas Integra 800 by the method of absorption spectrophotometry. Measured laboratory values were statistically processed and they were interpreted after consultation with the supervisor of the Bachelor´s thesis. The laboratory tests showed that the criteria of metabolic syndrome applied for 21% of women and 15% of men.
